If you are a student, do not complete the online registration form; please download the pdf and complete it, indicating that you are a student. Then send the completed form without payment to the address or fax on the form. You may also email it to email@example.com.
To register, please complete and return the registration form with payment to the Continuing Medical Education office address listed at the bottom of the registration form. You may also register by faxing your registration form to the CME office at 573/882-5666 or by completing an online registration.
Payment Methods: Fees may be paid by cash, check, or any major credit card (Discover, MasterCard, Visa or American Express). The check should be made payable to the University of Missouri. To pay by credit card, please fill out the appropriate section of the registration form and sign where indicated.
Substitution and Cancellation Refund Policy: A full refund of fees less a $25.00 administrative fee will be made if notice of cancellation is received, in writing, by Friday, October 20, 2017. No refunds will be made after October 20, 2017.
